Data Request Procedure

1. Click here to Complete the Cancer Data Request Form
2. Download, complete and submit the Data Release Agreement Form

Following completion of the Cancer Registry Data Request Form and Data Release Agreement Form:
1. A notification message confirming the data request will be sent to the E-mail address provided on the Cancer Registry Data Request Form.
2.The data request will be forwarded to the appropriate cancer registry director who gives approval for the release of the registry data requested.
3. On approval from the cancer registry director, requested data will then be sent to the E-mail address provided.
